Targeted Case Manager/Group Care Assistant – Full time
Partnership for Children (PFC) works with families and children, many of whom are impacted by trauma, poverty, and various social determinants of health, such as food insecurity, homelessness, and lack of affordable childcare. PFC’s trauma informed approach is based on asking, not what is wrong with you, but what happened to you? The attachment framework puts relationships at the center of treatment and creates a sense of security, connection, and belonging for children and their families.
The Targeted Case Manager (TCM) acts as the principal interface between the child and outside entities, providing effective leadership and advocacy in building effective relationships between treatment teams, families, and external stakeholders. A high degree of interaction is a major job aspect, requiring tact, judgment, and the ability to enter different diverse communities and influence a wide variety of individuals. The TCM is a principle participant in the process of referrals, assessments, intakes, admissions, treatment planning, family involvement, ongoing authorization, and discharge; and, has the primary responsibility for completing reports, maintaining records and documentation of these processes. As a key
interface between the child and outside entities, the TCM takes a lead role in coordination of care. The TCM plays an integral part in achieving integration of all treatment efforts by working with team members, families and other stakeholders to generate and sustain a clear direction of care for the child. As the Group Care Assistant, this position will receive, process and present referrals for Therapeutic Group Care.
